A response to a young man named Menachem Mendel who was offered a shidduch with a girl whose father’s name was the same as his:
לכתבו בענין השמות – יוסיף לעצמו (בעלי’ לתורה, בחתימה וכו’) שם בלה”ק [=בלשון הקודש] ומכאן ולהבא יעשהו לעיקר (אף שלפעמים יוכל להשתמש בשלושתם יחד או בהעיקר ואחד מב’ השמות דעתה).
ועוד ועיקר יתנהג כמתאים לתמים וה’ יצליחו.
Translation:
Concerning what you write about the matter of the names of you and your prospective father-in-law being the same:
Add for yourself (through getting called up to the Torah, through signing, and the like) an additional name in lashon hakodesh and going forward, this should be your primary name (even though occasionally you will be able to use all three names together or the primary added name along with one of the current names.
Additionally, and this is the main thing: act as a tamim in all matters and may Hashem grant you success.
